New Client Registration

If you wish to register as a client please fill in this form.  Please indicate if you wish to also book a visit and the best way to contact you to confirm.  We require a deposit of £95 before your first treatment which will be deducted from your first invoice. The reason for this deposit is to avoid people registering, using us once then never paying their bill.  This may sound surprising but it used to happen to us regularly before we introduced the deposit.
